Kindergarten Health Requirements

Dear Parents/Guardians,

Welcome to Montecito Union School! Before your child enters kindergarten there are a few things that we need to have on file. According to the CA Health and Safety Code, the following must be completed and presented to the school:

 1.     Complete Immunization Record (CA Health and Safety Code, section 120325-80)

  •  4 doses of Polio vaccine (3 doses meet the requirement if the last dose was received after the 4th birthday)
  •  5 doses of DTP - diphtheria, tetanus and pertussis vaccine (4 doses meet the requirement if the last dose was received after the 4th birthday)
  •  2 doses of MMR – measles, mumps and rubella vaccine on or after the first birthday
  •  3 doses of Hepatitis B vaccine
  •  1 dose of Varicella (chicken pox) vaccine or documentation that the child has had chicken pox

The school nurse will be available at Montecito Union School to review and copy immunization records and make recommendations if additional immunization records are needed.

  2.     Oral Health Assessment  (CA Health and Safety Code, Section 124085 – form enclosed)

California law now requires that your child have an oral health assessment by a licensed or registered dental health professional. Oral assessments that have been done within 12 months before your child enters school also meet this requirement.

Sincerely,
